One of them gives you the ability to create an ICS-214– If you DONT have a signed copy of your own you probably want to use that, fill it in, and SAVE on your computer for manual signing later after you print it…. When you have a signed, PDF, Then use the other link that allows you to SUBMIT — you fill out a bit of meta-data (see instructions below) and then upload the form you had saved. This should make it pretty easy, OK?

SIGNED COPY: You will need if possible to use the WORD or ODT form that is created with your SIGNATURE bitmap already in there. We made these last year, but EARL MCDOW may be able to help you if you can email him a cell-phone-photo of your signature.

ICS-214’s are uploaded to correspond with OPERATIONAL PERIODS — running from 7AM to 7PM. So make yours fit that, ok?

INSTRUCTIONS: When you click the filing link, you will have to fill in several boxes: (1) Your NAME; (2) your EMAIL ADDRESS; (3) the DATE you are actually filling in the links; (4) Employing Agency (Board of County Commissioners); (5) Incident (they will have this set for the current incident) ; (6) Incident Role — select ESF2; (7) Operational period – pick the correct 7-to-7 period for the report you are submitting; (8) Hours worked – put the number of hours you were involved (not including sleeping). Then use the “upload” area to upload a PDF of your ICS-214; select YES on the part where it asks if you have signed it, and then select SUBMIT. That should take care of it!
